Position Summary

The Community Learning Disability Team (CLDT) North Tyneside is part of Northumbria Healthcare NHS Foundation Trust. It is a multi-disciplinary team and is managed in conjunction with the services offered by the Local Authority.

This role will develop the current CLDT service to enable delivery of children and adult specialist service providing evaluation, intervention and training for people with a learning disability, complex behavioural requirements.

This role has a range of clinical activities including support to senior nurse with monitoring of behavioural case load, assessments and interventions and managing risk in the community. Liaising and working in partnership with the multi-disciplined work force.

The successful candidate will be passionate about working together with service users, their family, carers and professional colleagues to improve health and well-being and to prevent or minimise conduct that challenges.

Main responsibilities of the role

This role will be located within the Community Learning Disability Team in North Tyneside. The post holder will work within the Enhanced Crisis Avoidance Team. The postholder will contribute to and support highly specialised behavioural assessments and interventions for children and adults presenting with severe behaviours that challenge.

The job holder will support senior staff to build highly specialist Positive Behavioural Support Plans for patients and service providers to reduce or prevent placement breakdowns or admissions into inpatient institutions.

Job Overview

An wonderful opportunity has arisen for a dedicated and compassionate individual to join the Cellular Therapy team at Derriford Hospital. An opportunity to be innovative in supporting people afflicted by cancer, to walk beside them at some of the most significant stages in their care journey.

You will support the delivery of the Personal Care in Cancer programme, working closely with our Support Specialists/Clinical Nurse Specialist team. You’ll be a trusted source of information, emotional support, and guidance to programs that can make a real difference to patients and their families.

Excellent communication and organisational skills will be needed to succeed here, as you will be coordinating patient paths and maintaining links with the Haematology team, Mustard Tree, GPs, patients, relatives and other Trust workers.

Having a proven awareness of the issues experienced by persons living with a cancer diagnosis will be invaluable, but complete training and continuous support will be given. Previous NHS experience would be a plus but not needed – we are far more interested in your empathy, your initiative and your commitment to high quality care.

Main tasks of the position

This is a patient facing role that supports patients and carers through cellular therapy treatments. They will need to collaborate closely with the cellular therapy team to deliver high quality, person-centred assistance. This involves providing support to patients and carers during treatment and living with cancer. This could be at the clinic, on the wards visiting patients or in the day case unit. The role will involve engaging with other agencies that provide support, assisting the Benefits Advisor in completing grants and applications as necessary. There will also be an element of data collecting and statistical report writing to monitor patient activities and outcomes over the pathway.
