The position’s main duties are

Key duties:

To deliver the Trust’s Light Goods Vehicle to clinic venues and other locations within North Wales and occasionally other areas of Wales.

2. Community Learning Disability Nurse/Care Co-ordinator

Job Description

We are looking for a Band 5 Community Learning Disability Nurse to join an established Integrated Team. The team consists of a variety of professionals including Social Workers, Community Learning Disability Nurses, Psychiatry, Psychology, Physiotherapy, Occupational Therapy, Speech and Language Therapy and Clinical Nurse Specialists. Durham County Council are the primary authority for the Integrated Learning Disability Team.

The main recording system will be Azeus via Durham County Council with access to TEWV recording systems and NHS mail. The team work together, collaboratively and collectively with the wider MDT, to ensure a high standard of care is provided and ensure people with a diagnosed learning disability and their families and carers are supported by delivering high quality clinical care utilising the Care Act and TEWV clinical Pathways, commissioning packages of care to meet assessed needs and signposting to community services.

The service is now Monday to Friday.

Main responsibilities of the role

The selected candidate will be passionate about improving the mental health and well-being of adults with a learning disability and challenging behaviours. They will work within a multi-disciplinary team (MDT) offering high quality secondary care and improving community services, working with people who require specific interventions, using trauma informed care and a recovery-based approach.

Must be self-motivated, adaptable and able to communicate effectively. You must be able to travel freely and have access to a suitable vehicle for work needs. Adjustments may be considered if appropriate in line with the Equality Act 2010.

Key responsibilities include, but are not limited to:

Management of a diverse caseload including nursing assessments and interventions. Deliver therapy and interventions, monitor and evaluate outcomes.

Care Act assessments and reviews

Signposting to community services

Commissioning and evaluation of care packages

Cutting back on restrictive tactics

Multi-disciplinary working for improved outcomes for the people we work with.

Good communication and documentation skills producing reports and information in approved deadlines.

Meet legal frameworks/ policies and procedures e.g. Care Act Safeguarding, Mental Capacity Act and Mental Health Act.

Maintain their NMC Registration/ PIN guaranteeing the compliance with the NMC Code/ revalidation

This will include young individuals with anti-social conduct, mental ill health, learning difficulties or substance addiction to support rehabilitation & well-being. This is a fantastic opportunity for a Band 4 Nurse Associate to join our team in the Surgical Short Stay Unit.

Patients undergoing surgical operations will be admitted to The Surgical Short Stay Unit and patients having day case procedures will be discharged later from the same unit. The Nurse Associate will be responsible for the admission and discharge of patients in accordance with their surgical care plan and under the direction of the nurse in charge. Staff will be rostered on to the surgery Short Stay Ward to care for surgery patients throughout their intended post-operative overnight stay.

We are searching for a good team player and someone who has initiative and excitement for the position. We are also searching for someone who puts patients at the centre of all they do and who delivers great care throughout the peri-operative journey.

The post holder will be expected to provide high quality, patient orientated, compassionate care under the leadership of a Registered Nurse with an emphasis on encouraging health and independence. They will be a responsible professional and have the ability to work without direct supervision and at times delivering care independently in line with the individual’s defined plan of care, within the parameters of practice of the Nursing Associate role, accessing clinical and care advice as required.

The Nurse Associate will admit and discharge patients and ensure all information is given to patients and their relatives to facilitate their recovery. The Nurse Associate will work closely alongside their Registered Nurse and Clinical Support Worker colleagues to ensure patient flow across theatres is effective.

Develop and build on practical and theoretical knowledge, competence and abilities across the Nursing Associate Role and retain all evidence needed

Communicate effectively with a wide range of people, the public, health and social care professionals, making sure communication is focused on delivering and improving health and care services

Demonstrate inter-personal abilities that foster clarity, compassion, empathy, respect and trust
