Job Description

We are looking for an eager and driven Project Manager to join our Integrated Care Operations Service (ICO).

This is an amazing opportunity to work at the core of new integrated care projects, supporting service delivery, operational improvement, data driven decision making and partnership working across health, social care and voluntary sector groups.

You will be working directly with clinical and operational leaders to help shape and improve all ICO services including complicated discharge, Inclusion Health patients, UCLH Hospital@Home as well as high intensity users by providing project support and data function across the services.

This post provides the opportunity to work within a fast-paced and supportive team dedicated to delivering new and integrated services. You will have the opportunity to experience a range of operational, strategic and partnership work, whilst contributing to meaningful service change and improvement across the health and care system.


Key tasks of the role

The function is to assist the delivery and development of integrated, person-centred services across health, social care and voluntary sector partners notably those with complex needs such as care needs, homelessness, mental health difficulties and drug dependency. The postholder will work in conjunction with multidisciplinary teams to develop coordinated care pathways and promote inclusive health concepts, acting as a representative of the service in partnership forums and ensuring good partnerships across acute, community and external agencies.

The post will be responsible for the management and co-ordination of the administrative function, managing workloads and providing line management, supervision and development for staff from a leadership and operational viewpoint. It’s about working out meetings, helping teams communicate well together and building a healthy, values-based team environment. The postholder is also responsible for operational performance, risk management, service delivery support and escalation of concerns to guarantee highest standards of governance and operational resilience.

The Integrated Care Operations (ICO) Department is part of the Emergency Services Division and includes the following services:

  • Camden Adult Pathway Partnership (CAPP)
  • Homeless Intermediate Care Team (HICT)
  • Rough Sleeper Outreach Nurse (part of the Homeless Health Outreach Project we operate with North London Foundation Trust)
  • Integrated Care Operations admin team
  • Inclusion Health Service
  • Integrated Discharge Service (includes Complex Safe Discharge team and Admission Avoidance)
  • Alcohol and Drug Liaison Tobacco Dependency Service (ADLTDS)
  • UCLH@Home and UCLH BloodTransfusion@Home

The Department works with clinical colleagues throughout the Trust to engage with Integrated Care Boards, Local Authorities and community providers on the creation of new models of care. We also work alongside our internal Emergency department and Patient Flow colleagues to reduce duration of stay whilst promoting safe discharge.

We are searching for a Project Manager to support and drive improvements and innovation to all the ICO services, working with the existing Senior team.
