About the position:

We are looking for the right person to join us as a Care Coordinator and this is an excellent opportunity for a passionate and energetic person. The role is open for Nurses, OTs and Social worker.

The position holder will carry out assessments of newly referred persons to the Early Intervention Service who are exhibiting First Episode Psychosis (FEP). They will perform the role of Care Co-ordinator under the Care Programme Approach for a specified caseload of service users and their carers. They will provide evidence-based psychosocial therapies with an emphasis on engagement, recovery and social inclusion, as well as tackling physical health care needs.

The successful candidates will be experienced registered professionals who can demonstrate a specific interest in working with patients with developing psychosis and will have completed preceptorship or proof of band 6 abilities.

Key Job Responsibilities

1) Assessment

To be responsible for the assessment of newly referred people with First Episode Psychosis and the subsequent presentation of findings to the entire team.

To work with diagnostic ambiguity in providing continuous comprehensive assessments and result monitoring for all service users within caseload.

To do periodical risk assessments with any other health professionals/agencies involved.

Ensure risk assessments are reviewed often, are meaningful and are reflected in care plans.

2) Engagement

Take initiative to engage even the hard to reach clients.

To meet regularly with service users at a venue of their choice as far as possible, be this at home, CMHT centre, education, work or recreational establishments to assist with engagement and encourage participation in treatment delivery as well as facilitating independence in all aspects of daily living.

3) Support

To offer continuing care coordination for a specific caseload of service users who will be having their first episode of psychosis.

To offer support and advise in relation to service users with reference to the issues affecting them.

Work together with service users focusing on a recovery strategy.

Give information so clients can make informed choices.

Specification of the Person

Qualifications

Key

  • Appropriate professional qualification. e.g. RMN, CQSW or equivalent
  • Current Registration.
  • Evidence of continued professional development e.g. relevant courses.

Prefered

  • Meridan/FI training or similar

Experience

Key

  • Extensive experience in working with service users with psychosis at an acute phase of illness.
  • Demonstrated experience and skills in risk assessment and management.
  • Extensive experience and talents in clinical assessment.
  • Experience of dealing with service users with psychosis and their caretaker.
  • Experience of working in conjunction with statutory and voluntary services.

Looking for

  • Experience of the utilisation of evidence based interventions in relation to the management of service users with first episode psychosis.
  • Experience of working with service users experiencing psychosis in a community environment.
  • Experience with family interventions.
  • Experience with care coordination/CPA.

Talents

Necessity

  • Crisis management knowledge and experience.
  • Strong verbal and written communication abilities.
  • IT/data entry skills.
  • Ability to engage service users and carers in care planning.
  • Providing psycho education to service users and their carers about psychosis.

Preferred

  • Care co-ordination / CPA skills.
  • CBT/FI Competencies.
  • The implementation of evidence based treatments in the care of service users with first episode psychosis – experience.

Information

Essential

  • Understanding of the Recovery Approach and its use in practice.
  • Understanding of the Early Intervention in Psychosis Approach.
  • Knowledge of CPA, Community Care Act, Carers Act, Mental Health Act and other applicable legislation in relation to practice.
  • Understanding of the function of a Care Co-ordinator.

Looking for

  • Knowledge of current Government Policy and Guidelines in relation to provision of community mental health care and of EI in psychosis in particular.

DBS Check (Disclosure and Barring Service)

This role is exempt under the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (previously known as CRB) to check for any previous criminal convictions.
