POSITION SUMMARY

The position holder will be a member of the Enhancing Lives Team and will deliver support and wellbeing interventions to service users within the community.

The Support Worker will play a key role within the Enhancing Lives Team; to support the outcomes of engaging/re-engaging vulnerable individuals with complex needs in relation to issues including substance use, homelessness, mental health and offending behaviour, often underpinned by trauma.

Key responsibilities of the role

Support Workers will perform duties and roles delegated by senior team members.

The role will be working within the scope of the Enhancing Lives Team which allows the post holder to make a limited evaluation of individual need and act upon their assessment.

The post holder will work together with the multi-disciplinary team to address the needs of people being supported providing quality evidence based care.

This will involve working with cross-functional application teams to ensure the integrated functionality of the system and compliance with legal and regulatory frameworks.

The successful candidate will have the ability to work independently to fulfil project deadlines.
