The GNA Care Coordinator will help figure out what health needs a person has and help administer and evaluate care interventions that are made to suit those needs. This will be done under the direction of a senior registered nurse or practitioner. They will keep an eye on the care, try to cut down on care packages as much as possible, help with nursing and therapy rehabilitation and care plans, and generally help younger GNAs. Supervision can be direct or indirect, depending on how hard the work is and how skilled the person is. They will be in charge of the Buddy procedure for GNAs on duty, even when they are not working, in line with the Lone Working Policy.
A big part of the job is also helping patients near the end of their lives in their own homes, making sure that they and their families get the best care possible at this difficult time. This will mean looking after patients at the end of their lives and collaborating with Palliative Care in Partnership IPOCH Nurses and Practitioners, as well as other care teams, like district nurses.
