Responsibilities
Help people to design and implement individualised support plans that foster independence, community presence and meaningful participation.
Work with people, families, advocates and professionals to help them communicate effectively, make decisions and arrange support resources.
Help people to secure their financial and tenancy position by supporting them with their personal finance arrangements, benefit entitlement and compliance with the tenancy agreement.
Identify and report hazards and take all necessary actions to ensure the health, safety and well-being of individuals and guests in accordance with housing partner and service agreements.
Engage in training and professional development to preserve and improve your particular expertise and offer a high level of service.
Always promote and uphold community participation and high quality support services making people feel valued and appreciated.
Actively follow and apply corporate rules, procedures and best practice including fire safety, health and safety and equal opportunities.
Participate in team meetings, supervisions and appraisals to promote collaborative working and maintain high standards of care.
Promote inclusiveness, equal opportunities and anti-discriminatory behaviours actively in all phases of service delivery.
Identify additional requirements of individuals and provide specific support to meet those needs.
Be prepared to undertake various duties as required to satisfy the demands of the service, displaying flexibility and dedication to organisational goals.
Give people the support they need, in a way that feels comfortable, dignified and independent.
Inspire and help people to learn life skills such as budgeting, cooking and managing their house.
Maintain up-to-date records of client progress, daily activities, and any changes in behaviour or health state.
Follow health and safety procedures to provide a safe, clean and supportive environment.
Assist persons with personal care and hygiene including encouragement or direct support as needed.
Help with giving, managing or reminding people about their medicines as outlined in their care plans and policies.
Provide person-centred care in a way that promotes dignity, independence and wellbeing.
Assist service users with daily living activities, for example personal hygiene routines and medication scheduling.
Experience or a strong interest in working with adults in a supported living environment, particularly adults with physical disabilities, sensory impairments, or mental health difficulties.
Compassionate temperament and a sincere desire to help others live independently and improve their quality of life.
Excellent communication abilities to effectively interact with clients, families and colleagues.
Good organisational abilities with the capacity to manage activities and correct paperwork.
Good in helping people to be independent and to decide for themselves about their lives.
Ability to remain calm, adapt to stressful events, and keep professionalism.
A valid driver’s license (recommended) to aid with appointments and community access support.
