We are now searching for devoted and caring people to join our team as a Health Care Assistant.
Summary of the Job:
As a Health Care Assistant, this role will be providing in-home care services to clients in the community.
You will be helping people with personal care, day to day living and medical needs.
This function will involve regular scheduled home visits to clients assuring their health and welfare and providing first class customer service.
Door-to-Door services are included in this position.
Main Responsibilities
Personal Care Assistance
Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, toileting and mobility assistance.
Health Monitoring
Regularly monitor clients’ vital indicators including blood pressure, pulse and temperature.
Document and communicate all major changes to the health care team.
Medication Assistance
Assist clients with taking their prescribed medications as outlined in their care plan.
Offer suitable paperwork and reminders to administer medications on time.
Companionship
Offer companionship and emotional support to clients.
Build an encouraging and helpful environment to enhance the quality of life for clients.
Transportation Assistance
Provide clients with transportation to medical appointments and other critical errands, as needed.
Care Plan Adherence
Follow individualised care plans and ensure that the services delivered are consistent with the clients’ medical and personal needs.
Record Keeping
Keep accurate records of client communications including daily activity logs, health monitoring notes and any observations needing to be relayed to the medical team.
Communication
To offer the best possible treatment, establish and maintain open communication with customers, family members, and healthcare professionals.
Household Assistance
Provide assistance to clients with light housekeeping, meal preparation, laundry and other tasks to help maintain a safe and comfortable home environment.
Qualifications
Previous experience in health and social care, home care or related field 1 year recommended.
Valid driver’s license and reliable transportation to travel to clients’ houses.
Good knowledge of personal care procedures and the needs of elderly or disabled people.
Ability to safeguard confidentiality and follow HIPAA regulations.
Great communication and interpersonal abilities.
Compassionate, client- and empathetic-minded, with a real want to help others.

Ability to manage a hectic schedule successfully and work trip to patient homes.
Problem Solving
The ability to meet the individual requirements and problems of each patient’s circumstance.

The job entails working alone but also collaborating with other health care professionals and family members.

Sponsorship may be provided based on employee performance and eligibility.
Must have 3-6 months of service with the company to be considered.
If the employee is meeting company standards and following all company regulations and procedures, the corporation may decide to grant sponsorship support.
