Position Summary
Responsible as the Care Coordinator for the assessment and co-ordination of care needs, planning and implementation of evidence based care to a specific set of service users and their carers within a defined catchment area/locality. To offer professional advice and support to other agencies and members of the multidisciplinary team and to serve as a role model and resource for junior/less experienced professionals.
Key Responsibilities of the Position (Care Co-Ordinator)
As a Care Coordinator you will be provided with a caseload of service users and a range of responsibilities including evaluation and coordination of care requirements. Once requirements have been determined you will be needed to plan and implement evidence based care to the service users and their carers. You will have the support and supervision of your Team Lead and as a Band 6 clinician you will be expected to supervise and resource junior or less experienced professionals.
You will be able to call on the interventions of your MDT colleagues such as Support Workers, Social Workers, Occupational Therapists, Psychologists, Employment Specialists, Family Therapists and Medical Colleagues.
