Summary
We are looking for caring and passionate Community Care Workers to join our team. The ideal candidates will give high quality support to clients in their own homes, ensuring their comfort, safety and welfare. The roles give an opportunity to make a real impact in people’s lives by offering personalised, personalised care that is designed around each person’s needs. Care Workers will work closely with care plans, communicating effectively with clients and team members to promote independence and dignity.
NurseGate UK has been awarded a contract to deliver Community Care Services from September 2026 and selected applicants will commence employment in early September 2026.
Responsibilities
Personal Care Services:
Assist clients with personal care needs, including bathing, clothing, grooming and toileting in a way that promotes client dignity and privacy.
Aid in movement and transfer, keeping the safety and comfort of the clients at all times.
Assist clients with eating and drinking, including meal preparation and feeding as needed.
Assist with medication administration, making sure meds are taken as recommended and within business regulations.
Daily Living Help:
assist clients with household duties including cleaning, laundry, and shopping to assist them maintain a clean and safe environment for living.
Encourage clients to participate in social and leisure activities, hobbies, trips and community involvement.
Assist clients in managing their finances including paying bills or maintaining budgets as appropriate.
Ensure the safety and security of clients’ homes, noting any potential threats or difficulties that could impair their wellbeing.
Monitoring of health and wellbeing:
Observe and report any change in health, condition or behaviour of clients and communicate any concerns to the appropriate health care experts or family members.
Assist in implementing care plans, working with healthcare providers, social workers and family members to ensure that care is aligned with the client’s needs.
Provide a safe and supportive environment, assist with health disorders such as diabetes, dementia or mobility issues as outlined in the care plan.
Emotional and social help:
Provide companionship and emotional support to customers to help alleviate feelings of loneliness and social isolation.
Encourage clients to stay socially connected and involved in their communities.
Encourage clients to decide on their care, lifestyle and routines to encourage their independence.
Reporting and Record Keeping:
Keep accurate and up-to-date records of care provided, including daily logs, prescription administration and any changes in the client’s condition.
Inform the Care Manager or senior staff of any occurrences or concerns in accordance with business procedures.
Ensure all care paperwork is done accurately and on time, and in accordance with regulatory standards such as Care Quality Commission (CQC) rules.
Security and Compliance:
Adhere to all relevant business rules and procedures including those relating to safeguarding, health and safety and confidentiality.
Be alert to the safety and welfare of clients and report any safeguarding concerns or possible dangers to management.
Provide personal care and support in a respectful, professional and non-discriminatory way, in accordance with the principles of equality and diversity.
Communication and team work:
Work cooperatively with other care workers, family members and health professionals to deliver holistic and coordinated care of clients.
Participate in team meetings and training to enhance your knowledge and skills.
Effectively communicate with clients and their families, making sure their wants and preferences are understood and respected.
Requirements
Own car and a valid driving licence.
Previous experience of working in a care facility or delivering homecare ideal.
Understanding of care plans and the capacity to carry them out efficiently.
Good command of English (spoken and written) for effective communication.
Strong communication abilities, capable of developing trust with clients and co-workers.
Caring approach, real will to help needy people.
Ability to work as a team and individually.
Ability to adjust to changing needs of clients and schedules.
