Overview of the Job

This position supports the provision of community-based diabetes education, assisting people in gaining the knowledge and self-assurance necessary to manage their illness. You will work with a multidisciplinary team to provide interesting one-on-one and group sessions, encourage healthier lifestyle choices, and increase local populations’ access to education.

Additionally, you will monitor results, support referral routes, and contribute to service development—all of which will be crucial to enhancing diabetes treatment and patient well-being throughout the area.

Primary Responsibilities of the Position

  • Conduct one-on-one and group diabetes education sessions in community settings.
  • Encourage people to learn self-management techniques and make wise lifestyle decisions.
  • Encourage the implementation of structured programs like DESMOND and other educational modules.
  • Oversee referrals, prioritise patients, and plan educational activities.
  • Track, document, and report on program efficacy and patient outcomes.
  • Direct patients to the proper community, social, and health services.
  • Collaborate with community partners and interdisciplinary teams.
  • Participate in patient-centered treatment, quality enhancement, and service development.
  • Encourage diabetes education and training in all community services.
  • Speaking Welsh is preferred for this position, although both Welsh and English speakers are encouraged to apply.
