The States of Guernsey need a Support Worker to work with the Community Living Team.
This job helps persons with severe and multiple learning difficulties in a community living program that is open 24 hours a day.
People who live in their own homes or in special care accommodation get help through outreach and in-reach. Staff use the Active Support Model, which focuses on doing things with people instead of for them. They do this to help service users participate in daily tasks like personal care, managing their medications, cooking, cleaning, shopping, managing their money, and getting to know their community.
Band 4 staff must be able to work on their own and with others while following the directions of senior staff to give good physical and emotional care. They are also key workers who follow the service’s principles and quality requirements.
The job also involves working alone, especially when doing outreach visits or helping people in their own homes. This means that workers must be able to work securely, independently, and confidently while following company rules. They also need to provide 1:1 supervision to Band 3 Support Workers.
Managers may give you more work to do. Staff must be willing to drive service vehicles and have a valid driver’s license.

A big part of the job is helping patients, their families, consultants, nurse specialists, and other staff members with paperwork and coordination. The person who gets the job will be able to communicate well and be willing to keep learning and growing. Also, the person in this position will be able to point patients and carers in the right direction for the help and information they need.
The job’s main duties are
You will be able to figure out each patient’s information needs and make sure that all patients can get to the Holistic Needs Assessment and Care Plan. You will also be attentive to figuring out what each patient needs, such as benefits guidance, family assistance, and advocacy help.
You will help set up and take part in project steering committee meetings. You will help with collecting and keeping records, including patient experience metrics for audits and project evaluations.
Applicants should be able to read and write well, communicate well, use computers well, and organise well. They should also be willing to work in a flexible workplace. It is also important that you have worked with cancer patients or other long-term, life-changing diseases in a clinical setting before.

Creative, new, and breaking the stigma
In the Rough Sleepers Mental Health Project (RAMHP), we are looking for a Band 4 Peer Support Worker to work three or four days a week for a year.
RAMHP is a service that helps persons who sleep rough get the necessary mental health care by working with street outreach services. The service is available in Tower Hamlets, Hackney, the City of London, and Newham. We work with government and partner organisations that help those who are having a hard time. The team takes an assertive outreach technique, which means they go to see individuals where they are.
The Peer Support Worker will have been homeless and used mental health services before. You will talk about your personal experiences to build trust and give people hope that they can get better from someone who understands.
The main tasks of the job
This job includes knowing what rough sleepers need in terms of health care and the problems homeless persons have when they go to the doctor. You will help folks go through health and rough sleeping systems and get over the things that keep them from getting health care and other help.
You will work with people from different fields and go out with them. We go to where individuals are, which could be on the streets, in emergency housing, or at certain homeless day centers. You would be going through Tower Hamlets, Hackney, the City of London, and Newham. The job would involve talking to people who could have mental health problems and helping both us and them understand their mental health better. They might not trust services, yet they still need to be linked to the right help.
