The Adult Community Review Team helps adults live well and on their own.
The Adult Community Services Review Team helps persons with long-term and complicated needs live as independently as they can.
We work directly with customers in a person-centred way, doing Care Act 2014 reviews to learn about their needs, strengths, and objectives. This helps us make sure that their ongoing care packages are still suitable and successful. We use a strength-based approach that focuses on health, independence, and helping people reach their own goals wherever possible.
As a Social Care Practitioner, you will get a lot of help from experienced Social Workers and Social Care Practitioners, and you will have chances to learn and grow all the time.
One of your main jobs is to do annual reviews and reassessments that are centred on the person and their goals. You will check on care both in person and over the phone, and you will update support plans to make sure they meet the needs of each customer. So, travel is a big aspect of the job.
You need to be able to work well with others, including clients, families, and teams of professionals from different fields. This job moves quickly, therefore it’s necessary to be well-organised and able to handle a lot of cases with confidence. You must have experience with Adult Social Care processes and be able to work quickly and accurately. It would be good to have an NVQ Level 4 in Health and Social Care (or something similar), or be prepared to work toward it.
We are now looking for four full-time equivalent employees and would love to hear from you if you desire a rewarding job that will help people be healthier and more independent.
This job requires you to work all around the county to help with service delivery.
