Overview of the Job
One person’s function, task, or obligation is not care coordination. A smooth care experience is produced by the integration of services, coordination, information sharing, and communication among carers, treatment providers, individuals with and without cancer—known as tailored care and support—and their families. In addition to working with the larger clinical workforce to support the implementation of the Personalised Care and Support (PCS) agenda for all cancer patients with the trust, the position holder will report to the Lead Cancer Nurse and be in charge of operational management and leadership of the Cancer Support Worker (CSW) team.
The position holder will support the Personalised Care and Support (PCS) plan in collaboration with the Lead Cancer Nurse.
To ensure equity for all patients affected by a cancer diagnosis, the position holder must coordinate and support the implementation of cancer support services across all tumour sites in collaboration with the Lead Cancer Nurse.
The Cancer Support Workers will be line managed by the post holder.
Principal responsibilities of the position
The position of Cancer Support Worker Team Lead will entail overseeing a group of cancer support workers, assisting the MDT in encouraging patient self-care, and helping cancer patients return to as normal a life as possible after diagnosis and treatment.
To help the Lead Cancer Nurse and Lead Cancer AHP understand the service’s future prospects, the position holder will conduct a scoping exercise.
