Overview of Job

This is a great role to build your skills and expertise in caring for older persons with frailty requirements. The role will have a focus on enhancing the patient experience through enabling independence and supporting patients to retain and maximise functional capacity.

You will be needed to encourage and help patients with mobility, nourishment and hydration and support them to participate in daily interactive activities that contribute to their overall well-being.

The post holder should possess an NVQ Level 3 in Health (or equivalent). They should be able to communicate effectively, work well in a team, and also be able to operate on their own as required. You will work closely with the nursing staff and the therapy team to give high quality, patient-centred care.

Main responsibilities of the role

Deliver High-Quality Patient Care

Help give high quality care to older persons with frailty needs, with a focus on improving the overall patient experience.

Support Independence and Wellbeing

Encourages and maintains patient independence encouraging participation in mobility, nourishment, hydration and daily interaction activities.

Personal Care Support

Assist patients with personal care such as washing, dressing, toileting and ensure patients dignity at all times.

Safe Mobility Assistance

Promote safe mobilisation and use of appropriate equipment and follow moving and handling guidance.

Monitor and Report Patient Conditions

Report changes in the patient’s condition promptly to the registered nurse or treatment personnel.

Clinical Support Duties

Carry out delegated clinical duties, such as observation, fluid and food monitoring and reporting as needed.

Multidisciplinary Team Collaboration

Collaborate with nursing team, therapies team and larger MDT to achieve customised patient goals.

Welsh Language Skills

Welsh language skills desirable for this role; applications are welcome from Welsh and English speakers alike.
