Job overview

We are looking to employ a motivated Health Care Assistant with a real interest in providing safe and effective care for patients in their own home. They will have demonstrable experience, a wide range of clinical abilities and will be confident to operate without direct supervision. Community nursing is growing fast and you need to be able to work positively with service improvements and change.

Main duties of the role

The post is to serve as part of a Community Nursing Service providing high quality care to patients in their own homes in accordance with individual care plans. Provide personal and palliative care to ensure patients are treated with decency, respect and compassion whilst supporting their comfort and wellness. Demonstrate capacity to operate independently and autonomously in the community environment. Effectively manage tasks and maintain a high standard of care. Support the Community Nursing Team with the ordering and organization of clinical supplies and stationery to ensure services run smoothly. Adhere to Trust policies, procedures and professional practice at all times including health and safety, infection prevention and control guidelines at all times. Always protect patient confidentiality and handle information carefully and responsibly in compliance with legal, ethical and organisational obligations. Contribute to a safe, caring and professional atmosphere of care for patients receiving community based nursing services.

Purpose of the Job:

To support the Community Nursing Team in the delivery of nursing care and support to patients and families in the community environment.

Duties / Responsibilities include:

To work as part of an integrated Community Nursing Service in the provision of specific care to a patient within the community setting as documented in the care plan.

To enable palliative patients to be cared for and supported in their own homes.

Able to work independently without supervision and autonomously.

Responsible for ordering clinical supplies and stationery for the Community Nursing Team

To work in line with Trust Policies and Procedures.

To maintain high standards of personal hygiene in conformity with Health and Safety Guidelines and Infection Control Guidelines within the trust.

To maintain confidentiality of any confidential information which may come to your notice in the course of your work and shall not disclose such information without the consent of the patient/client or a person entitled to act on his/her behalf unless required by law or by the Order of a Court or in the public interest.

Clinical.

To assess skin integrity of patients at risk of formation of pressure ulcers and provide basic preventative information.

To provide complete personal care to patients in the last stage of life.

To give health screening to patients at home e.g. Monitoring of blood glucose levels, phlebotomy, blood pressure monitoring and urine tests.

To offer clinical intervention for patient care as directed by Registered Nurses after training and assessment of competencies e.g. administration of insulin, basic wound care and bladder irrigation.

Regularly examine equipment to make sure it is safe to use.

To complete obligatory training as required by the Trust Policy.

To discuss topics for improvement within a Personal improvement Plan with the District Nurse co-ordinator/District Nursing Sister.

To report any Health and Safety or Infection Control problems.

To support clinical practice to the desired standard of NVQ Level III or equivalent.

Prerequisites

GCE/GCSEs Grade A*- C/level 9- 4 in Maths and English or Equivalent or Willing to study Level 2 Functional Skills Qualifications in Maths and English

NVQ Level III in Healthcare/equivalent qualification

Experience working in a healthcare environment demonstrable
