The Lancashire Early Intervention Service (EIS) provides support for people aged between 14 and 65 who are at risk of, or are suffering, first episode psychosis. We have a range of professional interventions available within the team including Cognitive Behavioural Therapy and Family Interventions in line with current NICE advice. There are three multi-disciplinary early intervention service teams across the Lancashire footprint.

The Lancashire Early Intervention Service is hosted by the Lancashire and South Cumbria NHS Foundation Trust’s Children and Families Network. We work closely with a number of community and inpatient services for children and adults.

We are committed to maintaining our focus on providing the best quality clinical service and to this end we place a significant emphasis on professional and clinical supervision to help and develop individuals.

Key responsibilities of the position

Job Description

Specialist Outreach Worker – Peer Inclusion Health:

We are looking for a dedicated and Specialist Outreach Worker (Lived Experience of Inclusion Health) to join the Find&Treat service enabling people living with HIV and other infectious diseases to engage with care.

The role will focus on working with individuals who are out of care or at risk of disengagement, particularly those who are homeless, have substance use issues, are vulnerable due to migration or contact with the criminal justice system.

As a Specialist Outreach Worker, you will utilise your lived or comparable experience to establish trust, boost engagement and assist clients overcome barriers to obtaining healthcare and wider support.

We are searching for someone who: o Has experience of living with, or a good awareness of, the problems experienced by inclusion health populations o Is confident in engaging with individuals in community settingso Has great communication and interpersonal skills o Is able to work flexibly and independently as part of a team o Is driven to enable people to improve their health and wellbeingExperience of working in outreach, peer support or health and social care settings would be desirable but not essential.

Main responsibilities of the role

Specialist Outreach Worker will: As part of a multi-disciplinary team

Help persons living with HIV to re-engage with care and attend clinical visits.

Build trusted ties with people who may be disenfranchised from services

Outreach within community settings, such as hostels, day centres and other places

Provide practical support to overcome barriers to care (e.g. finding your way around services, attending appointments)

Support in BBV Testing, TB & STI Screening, in partnership with clinical staff

Collaborate with nurses and other professionals to support continuity of care

Recognise larger health and social issues (e.g. housing, welfare, drug use, mental health) and help people to find relevant services

Accurately record information and contribute to service monitoring and evaluation

Contribute to a trauma informed and person centred approach to care

This is a generalist inclusion health outreach post with a heavy HIV focus. You will be working flexibly across London serving people in a range of community settings.

You will be part of a supportive multidisciplinary team and will be trained and supervised to build your abilities in outreach, engagement and health support.

Social work training and supervision is provided, along with possibilities for further development of intervention skills essential to operate in a community mental health team.

Principal responsibilities

The post holder will use the principles of social work through professional judgement, decision making and actions within a framework of professional accountability. They will lead and advocate for the social viewpoint and trauma informed social model of mental health and wellness within the multidisciplinary team. They shall respect and take account of the expertise of service users, carers and other experts.

The position holder will work within a multi-disciplinary team offering social work knowledge to enhance mental health outcomes for individuals by understanding the socioeconomic determinants of mental health. This comprises the completion of parts of work when a patient requires social work input/social work treatments; and care coordination for people facing social stresses that are impacting their mental health or social stressors are the major issue that requires support.

4. Senior Occupational Therapy Assistant: Sponsorship Available

We are a pleasant team, working together, supporting patient flow and providing a smooth transition between hospital and community, with good MDT working, and strong relationships with our community colleagues.

The post would suit someone with good communication and interpersonal skills who has experience as an Occupational Therapy assistant with the necessary certifications for this role at Band 4.

As an Occupational Therapy Technical Instructor you will have a clinical and ward based caseload in a rehabilitation setting, you will take lead on discharge planning and undertake access visits in the community under the supervision of a licensed occupational therapist.

Welsh language skills are required for this role.
