Summary of the Job
There is a great chance for a Band 5 Community Learning Disability Nurse to join an established Integrated Team that works with an MDT made up of Social Workers, Community Learning Disability Nurses, Psychiatrists, Psychologists, and more.
Physiotherapy, occupational therapy, speech and language therapy, and clinical nurse specialists. The Integrated Learning Disability Team is led by Durham County Council.
The candidates will mostly utilise the Durham County Council system Azeus for recording, but they will also have access to TEWV recording systems and NHS mail. The team works together and with the larger MDT to make sure that people with a diagnosed learning disability and their families and carers get the best care possible. They do this by using the Care Act and TEWV clinical Pathways to provide high-quality clinical care, commissioning packages of care to meet assessed needs, and directing people to community services.
The service is now only available on weekdays. This is a great chance to work for a service that puts patient care first, whether you’re a new or experienced Registered Nurse. You will get the time and help you need to make a significant difference every day.

The main tasks of the job
The person who gets the job will really want to help persons with learning disabilities and behaviours that are hard to deal with better their mental health and well-being. They will be part of a multidisciplinary team (MDT) that provides high-quality secondary care and improves community services. They will deal with people who need specialised help utilising trauma-informed care and a recovery-based approach.
Must be able to travel on their own and have access to an appropriate vehicle for work. They must also be motivated, flexible, and able to communicate well. According to the Equality Act 2010, changes can be made if they are needed.
The job’s main duties are, but are not limited to,
Managing a changing workload that includes nursing assessments and interventions. Giving therapy and interventions, keeping an eye on outcomes, and evaluating them.
Reviews and appraisals of the Care Act
Pointing people to community services
Ordering and reviewing care packages
Cutting down on restrictive practices
Working with people from other fields to make sure the people we work with get better results.

The Community Learning Disability Team (CLDT) is a section of Northumbria Healthcare NHS Foundation Trust in North Tyneside. It is a team of people from different fields who work together with services offered by the Local Authority.
This position will help build on the present CLDT services by providing a speciality program for children and adults that includes assessment, intervention, and training for persons with learning disabilities, complicated behavioural issues, or mental health needs.
