When a patient has a mental health crisis and would otherwise need to be admitted to the hospital, our crisis resolution and home treatment teams (CRHTs) are intended to provide safe and efficient care in the patient’s home.

In addition to facilitating a positive hospital discharge and easing the transition back to community-based services, we provide home visits, medical evaluations, psychological advice, and telephone assistance. We frequently serve as the liaison between our Acute Care and Community Mental Health teams, with whom we collaborate closely.

Support staff, nurses, social workers, occupational therapists, peer and carer support workers, mental health nurses, physicians, and psychologists make up our teams.
