To fill the position of Band 4 Senior Community Support Worker (Senior Access Facilitator) in West Sussex, we are seeking incredibly driven and passionate Physical Health Support Workers with prior experience working in a mental health context. Possessing a Care Certification would be advantageous, and you will be eligible to get an NVQ Level 4 Diploma in Health and Social Care or an equivalent degree.

As a key member of the Mental Health Multidisciplinary Team (MDT), you will collaborate with a number of general practitioners’ practices in the Bognor Regis and Chichester Primary Care Networks in a specific region of West Sussex to guarantee a comprehensive, coordinated strategy for patients with severe mental illness.

  • After completing venepuncture training, actively conduct community visits in a range of settings (such as home visits and G.P. surgeries) and provide a physical health examination that includes drawing blood.
  • To create physical health care plans and goals in collaboration with individuals and pertinent personnel in order to support and encourage physical health self-management.
  • To offer line management assistance, which includes daily oversight and assistance to B3 Community Support Workers (Access Facilitators).
  • In order to support sustainability, refer and assist those you work with in building positive relationships with important partner organisations and communities so they can access health improvement activities.
  • Build trusting connections with important stakeholders, particularly the community mental health team or teams and primary care providers, such as general practitioners, nurses, and managers.
