1. Clinical Care Co-ordinator/SCM Worker with Visa Sponsorship
NHS: Herefordshire and Worcestershire Health and Care NHS Trust
Date of Closure: January 12, 2025
Overview of the Job
The Structured Clinical Management paradigm is used by the Complex Needs Service (CNS) to treat patients with Complex Emotional Needs. This position involves working with a team that has ample resources, and your contributions will have a big impact on someone’s life-changing recovery.
An interesting opportunity has emerged for a current Band 6 member who wishes to advance their education and training through therapeutic techniques, or for an established Band 5 member who wants to take on a development role within our Complex Needs Service Team. After completing the skills and working for 12 months, the development program will end with a promotion to Band 6.
At the very least, you will receive foundational training in DBT. Peer support, outside training, and supervision will also be used to give SCM training. We will provide all the training needed for the suitable applicant to fill the position, thus you are not expected to apply with all the necessary knowledge.
You will receive weekly reflective practice, a protected peer-led workshop space, and a high degree of supervision, including clinical supervision and line management.
Principal responsibilities of the position
From assessment to stabilization, treatment, graduation, and return to the patient’s general practitioner, you will play a crucial role in the patient’s journey through the Structured Clinical Management treatment program.
You will see your patients once a week and build a strong therapeutic relationship, which will allow you to provide psychologically informed therapies within the framework. This is not your standard care coordinating function.
As they go through the course of treatment, your patients will also attend CNS Skills groups and, if necessary, extra meetings with occupational therapy assistants and peer support workers.
With the exception of bank holidays, you will work Monday through Friday from 9 a.m. to 5 p.m.
Ten patients will be the most cases you can handle.
You will get the chance to co-facilitate a group project, like the CNS Skills group.
We are a helpful multidisciplinary team with a wide range of expertise, and you can get support from a number of disciplines for any questions that come up during the day.
Regular clinical supervision, weekly group reflective practice, management supervision, peer-led workshop time, monthly continuing professional development, and daily huddles are all included.
In order to help you in your position and advance your career, you will be strongly urged to participate in training. This will involve formal training, peer support, and mentoring, such as MBT (including fortnightly workshops) and/or DBT Foundational Training.
