Working with patients who have been diagnosed with severe mental illness and cardio-metabolic risk factors would include collaborating with the LMHT and secondary mental healthcare services.
Serious mental illness patients typically pass away 15–20 years before the normal population, frequently as a result of inadequately identifying or managing their cardio-metabolic risk factors.
In addition to coordinating and communicating with the various service departments that the patients interact with, this position will entail collaborating with mental health teams and general practices to proactively identify and assist the patient in managing these risk factors.
