In order to ensure that clients receive care in their homes and are supported in leading independent and satisfying lives, we are seeking a committed Care Coordinator to join our expanding business and be a vital member of a busy and successful team.As you will be communicating with both health professionals and customers, you will exhibit exceptional communication and organisational skills. You will be ensuring the highest level of customer service to deliver a safe and effective service that satisfies both company and CQC requirements.Every day will be different because every consumer is different. Helping our clients live the lives they choose is our top priority.It is crucial that you are willing to assist our clients directly if and when needed.This position is weekdays, with occasional after-hours and weekend on-call responsibilities if needed.
DUTY The duties of a care coordinator include:
- Arranging our care staff’s schedules.
- Communicate rota changes to clients and coworkers in an effective manner.
- Providing rota-based support for the on-call service
- Create and preserve a good rapport with our clients, their families, coworkers, and other experts.
- Ensuring that all electronic documents and paperwork pertaining to customers and Care & Support Workers are maintained current.
- Making certain that all other documents are maintained current
- Make use of technology to provide high-quality healthcare.
- Make sure that safe working procedures are followed.
- Building trustworthy relationships with clients, their families, employees, and other medical professionals like district nurses, general practitioners, and occupational therapists is an essential.
- Making certain that employees possess the abilities, education, and skills required to do their jobs
- Setting up evaluations of care
- Conducting care package monitoring over the phone.
- Exhibit the ability to make decisions and solve problems.
- Outstanding IT abilities, especially with Microsoft Office (Word, Excel, PowerPoint, Internet Explorer, and Outlook).
- Capacity to build relationships with coworkers throughout the company
- Participate in the emergency on-call roster during off-peak hours.
- Being a member of the Care Team requires you to be able to work flexibly.

Knowledge, expertise, and experience
- Planning experience would be advantageous.
- Experience working in the social care and healthcare sectors
- Outstanding organisational abilities
- Capacity to function under pressure
- Make wise decisions and use sound judgement.
- You must be passionate about providing outstanding care and support, self-motivated, and enthusiastic.
- Access to a vehicle and a valid UK driver’s license are prerequisites.
- The capacity to supply references and clear a DBS check.
Pay: Experience determines a competitive compensation.

Salary: £22,880.00 to £23,355.00 year
