We are presently looking for a Care Coordinator who is excited and eager to join our growing team.
THE JOB
You will be at the center of our care delivery as a Care Coordinator. You will be in charge of coordinating care services on a daily basis, leading a team of care assistants, and making sure that every service user gets safe, effective, and person-centred care. This is a great chance for someone who really wants to help people to do something good.
MAIN DUTIES
Do initial care evaluations and make personalised care plans
Do full risk assessments and regular reviews of care
Lead, manage, and help a group of care assistants
Do spot checks and watch over caretakers directly
Give out and keep track of prescriptions according to policy
Make sure that every care is recorded correctly and on time.
Talk to clients, their families, and healthcare providers
Follow the rules for health and safety, infection control, and safeguarding.
Use your own automobile or a corporate car to drive between care visits.
WHAT WE WANT
Level 2 or 3 NVQ/Diploma in Health and Social Care (or something like it)
Experience working in a medical context
A real love for caring for others and helping vulnerable adults
Great talents in communication, organization, and managing time
A full UK driving licence (or a promise to get one within 12 months)
Willingness to go through our training and orientation program
WANTED (NOT REQUIRED)
Experience with giving medications and keeping an eye on health
Understanding CQC criteria and how to follow the rules
Proof of continuing professional development (for example, dementia care, mental health, etc.)
Wanting to get up in the care field

37 hours of work every week
