About the Role
The Community Response Team offers temporary and interim care to patients being discharged from acute hospitals.
- Prior experience providing care is a must.
- To cover the areas of Amber Valley and Erewash, Babington Hospital will serve as your base.
- You will need to be adaptable and dedicated to helping those who require care at home.
- You must have a valid UK driver’s license and access to a car because you will need to be able to travel between visits.
- Weekends and bank holidays are included in the seven-day shift schedule.

Main Duties
- To be in charge of overseeing Support Workers on a daily basis within the Community Response Team.
- Assign patient visits to make ensuring that patients receive timely, cost-effective support worker services.
- To actively monitor and uphold clinical skills and a professional demeanour while serving as an example for the Support Worker team.
- When appropriate and under supervision, optimise the possibility for rehabilitation by having efficient communication with patients and caretakers.
- Collaborate with certified nurses and therapists to evaluate and treat patients with a range of ailments, and advance or alter intervention plans in accordance with guidelines established by the nurses and therapists.
Qualifications
- Training and educational requirements (essential)
- NVQ level 4 or comparable expertise acquired via education and experience
- GCSE Maths and English
- Experience performing in a band Three Support Employees
- Experience in the community
- Experience working in a rehabilitation setting with adults and older people
- Competencies and qualities (essential)
- Driving a car or having the capacity to move between caseloads on a daily basis
- Experience and aptitude in manual handling
- Ability and character traits (crucial)
- Flexibility in meeting service requirements
- Capacity to move across a large geographic area (AV and ERE)
