Duration of Employment
In order to receive sponsorship, you must come to an agreement with the hospital regarding the duration of your stay, which can be as long as four years.
Submission of a Visa Application
Both your visa application and the hospital’s application must be filed at the same address; for example, if an electronic application is recommended, both applications must be filed using the same medium. The destination application centre must be the same whether the application is sent by surface mail.
The application must be submitted with payment for any applicable fees. The Form 990i-Fees and Charges document, which is available at this address, has this information on page four.
Applications for visas must be submitted in Australia or, if preferred, online via the Department’s website. You don’t have to be in Australia at that moment.
Dependents
If you have spouses or children who will be travelling or staying in Australia while working for Friendly Society Private Hospital, you will also need to provide their personal information, including their names and birthdate. Both you, the visa applicant, and the Nomination Sponsorship Application that we (FSPH) produce must include them.

- Certified copies of birth certificates or the family book with the names of both parents for each kid listed on the application. For any kid under the age of 18, certified copies of the paperwork confirming custody and access arrangements are required, unless both parents are listed on the application. Certified copies of all adoption documents are included with the application for an adopted kid.
- Proof of Qualifications
- References from prior employers
- A letter from the employer confirming that the dependents are covered by this nomination if they were not on the prior nomination application
- Proof of eligibility to work in the designated occupation based on registration.
