Community Mental Health Care Coordinator Jobs in the UK with NHS Visa Sponsorship 2026

Summary of the position

To make sure that people with mental illness who are referred to HillingdonCMHH have their mental health needs properly assessed, are supported and receive a high standard of quality care within the resources that are available.

To help and advise users, carers and other professionals about mental health difficulties.

Main responsibilities

  • Managing caseload, administering medicine, duty work and attending MDT meetings.
  • To carry out holistic assessments of service users through the integrated notes system (SystmOne) including the Single Assessment Process, Risk Assessment and Mental State tests.
  • To promote the work of the Hillingdon CMHH and mental health concerns impacting adults with mental illness.
  • Maintain up to date clinical records and documentation (including electronic records) in accordance with established local policy and procedures, NMC standards and CNWL policy.
  • Maintain personal professional practice and clinical expertise in conformity with NMC recommendations and CNWL policies and procedures at all times.
  • To work in conjunction with statutory and voluntary agencies and the primary care team to ensure that appropriate care needs are fulfilled.

We are service user directed and work with adults who have physical and mental health concerns requiring a comprehensive approach. We are currently in the process of transitioning to the new Hub model therefore this is an excellent opportunity to join the team at this time of change as services evolve to be more responsive to the service users within the Hillingdon borough.

We are a multi-disciplinary service consisting of mental health nurses, occupational therapists, psychiatrists, psychologists, job specialists and peer support workers. There is a robust administrative structure and a fantastic working atmosphere and the staff is supported. The team has good contacts with local statutory and non-statutory agencies.

Job description

Responsibilities

  • To carry out a full assessment of service users with mental health problems.
  • To support the work of the Community Mental Health Hub (CMHH).
  • To keep clinical records and documentation in accordance with established local policies and procedures, NMC recommendations and CNWL policy.
  • Respond to daily duty referrals and ability to prioritise urgent requests.
  • To maintain own professional practice and clinical expertise at all times in accordance with NMC recommendations and CNWL policies and procedures.
  • To work in conjunction with the primary care team and statutory and volunteer agencies to ensure appropriate care needs are addressed.
  • working with patients, carers and other team members to safeguard patients.
  • Responsible for detecting potential for or indicators of patient damage, abuse or neglect and taking all necessary actions to protect the patient.
  • Provides clinical consultation for complicated situations to other clinical team members and staff from different disciplines.
  • Conducts urgent and priority assessments including full risk assessments.
  • Responsible for decision making and coordination in a crisis situation.
  • Short term support service users who present to the team when their Care Coordinator / Lead Professional is not at work and the person needs an urgent response.
  • To act as a point of contact to liaise with other services i.e. Police, A & E, Single Point of Access.
  • To be involved in defining quality standards and to provide a consistent approach to open people within the team.
  • Maintain an excellent reporting level, verbally and in writing on patients conditions.
  • Provide support to team members on other expert assessments as required e.g. Mental Capacity Act 2005; Knowledge of Deprivation of Liberty Safeguards, s42 Care Act relating to Safeguarding Adults and Making Safeguarding Personal.
  • To provide professional levels of care at all times.
  • To supervise younger staff and support students on placement.
  • Complete and maintain all training required for the job role.

PERSONAL SPECIFICATION

Education and Credentials

Summary

  • Current registration as a Registered Mental Health Nurse/ Occupational Therapist or Social Worker.

Experience

Critical are

  • Experience post registration.
  • Experience in working with, or in, the community context.

Obtainables

  • Supervision of others’ work.

Skills and Know-How

Necessary

  • Ability to contribute to the work of the MDT band Participation in the work of the team.
  • Demonstrate a knowledge of the range of therapeutic assessment and interventions available to reduce distress for individuals experiencing an acute mental health crisis or with an ongoing support need (risk assessment, MSE, capacity assessment, Full needs assessments) and effectively manage a complex caseload.

  • Auditing and standard-setting.
  • Evidence based service development.
  • Teaching and supervising qualified and unqualified personnel.
  • Ability to assess and support people in acute mental illness distress and crisis.
  • Communication skills (verbal and non-verbal) and telephone skills.
  • Skills in a range of therapeutic interventions.
  • Psycho-social therapy options skills.
  • Provide clinical leadership.
